Graduate Admissions Application Checklist

ALL APPLICANTS

Please note that students are only admitted into the M.S. and Ph.D. programs for the fall semester. The department generally does not admit students for spring semesters.

To be sent to UF Office of Admissions:

_____  Completed application form and application fee of $30.00 (U.S. currency; check or money order)

_____  Official transcripts sent directly from your university to the UF Office of Admissions

  • Official test scores must be reported to UF directly by the Educational Testing Service.  If you do not specify the University of Florida as a recipient when you took the test, then you must contact the Educational Testing Service to request that your scores [test score code 5812] be sent to the university. If the scores are not available in the UF system by Dec. 1, 2019, please email unofficial score reports to Dr. Joshua Brown. Note that you cannot be admitted to the program until your official score is sent and received by the Office of Admissions.  
  • Copies of score reports via mail are useful to the department for informal evaluation, and it is a highly recommended (not required) addition to your packet. However, you cannot be admitted to the program or the university until your official score is sent and received by UF Office of Admissions.

Note: The GRE is no longer required for admission to the UF College of Pharmacy’s graduate programs. GRE scores will not be considered in admission decisions.

To be sent directly to the Department of Pharmaceutical Outcomes and Policy:

_____ Resume/CV (if not submitted through online application)

_____ Unofficial reports of your GRE and (for international students) TOEFL scores to Dr. Joshua Brown if your official scores are not available in the online UF system by December 1, 2019.

_____ Three letters of recommendation (if not submitted through online application)

Note: Submitting recommendations online through the application is encouraged.

  • If the online choice is not an option, recommendation letters may be mailed via traditional post directly to this department or via email (see below).

For Traditional Post Recommendation Letters

  • Letters must be on letterhead and signed on the bottom by your recommender.
  • Please have your recommender sign the back of the sealed envelope prior to having them mailed to this department.
  • Unsealed and/or unsigned letters of recommendation will not be accepted.

For Emailed Recommendation Letters

  • Emailed letters of reference will only be accepted if recommenders have trouble accessing the online application, or if the original sent by traditional mail has not reached our offices within two weeks after it was mailed.
  •  Letters must be sent as a PDF attachment, and should be a copy of the original letter. The letter must be signed by your recommender.
  • Letters must be sent from the recommender’s email address.  PDF letters originating from the applicant’s email address will not be accepted.

 

_____  Biographical sketch, including a statement of intent (if not submitted through online application).

  • The biographical sketch should explain how your prior experiences and academic background have prepared you for graduate study, how your interests overlap with those of the department, and your future goals.
  • Please limit this sketch to one or two pages.
  • It is preferred that you print and mail this document in with the rest of your application packet. However, you may also submit this through the online application.
  • Please do not send this to the Office of Admissions.

_____ An informal email to Dr. Joshua Brown to let our department know that you have applied to our program. This helps us look out for your application in the UF application system.

 

INTERNATIONAL APPLICANTS

In addition to the items identified above, please provide the following:

If English is not your native language, please provide to the Office of Admissions:

_____ TOEFL iBT (Test of English as a Foreign Language).

  • TOEFL scores are required for all applicants who are seeking funding; no exceptions.

PLEASE NOTE: College policy requires that all students must submit GRE scores and international students must also submit TOEFL ibt before any application will be considered for admission.  There are no exceptions to this policy.
